Information for Insurance Utilization

Clients are responsible for payment at the time of each session. If a client wishes to use insurance, appropriate forms must be signed and SVC will submit for direct payment to the clinic. Clients are responsible for payment until the agency has received the insurance reimbursement. Some plans require prior authorization. 

Fees & Payment

Payment is expected at the time of service. We require a 24-hour appointment cancellation notice, otherwise you will be charged a fee of $25.00.
